Pre-Admission Testing Center

Yale's Adult Pre-Admission Testing Center is dedicated to meeting the concerns of our pre-operative patients and their families. A visit to the Pre-Admission Testing Center gives each patient having surgery with subsequent overnight admission at Yale-New Haven Hospital, the opportunity to facilitate health history reviews, registration, and testing.

The Center is located on the first floor of the Hospital's East Pavilion. Your visit should be scheduled up to 30 days prior to your surgery or procedure. Appointments may be scheduled Monday through Friday, 7:30 am - 2:30 pm, by calling 203-688-6539 or 203-688-5860.

For your convenience, there is a link to our patient information form below. Please complete this form and bring it with you to your visit. We find it is beneficial for patients to complete the form in the comfort of their home so they may accurately record their patient information.
